PROBLEM:

RESULT:
Once all five entities transitioned to paperless charts, the staff stopped using paper forms almost immediately and the clinicians did not have to reduce the number of clients they could see.  Now client records are available simultaneously to all appropriate staff at all times, allowing staff to locate and process patient information much more efficiently.
